DOH Developmental Disabilities Division

If an individual is eligible for services (for all ages with a qualifying developmental disability), case managers assist individuals, guardians, and families to create change through a person-centered process based on the individual’s desires, dreams and goals. Case managers connect individuals to available resources in the community as well as HCBS Medicaid DD/ID Waiver Services to support these individuals remaining amongst family and friends and living as productive, contributing members of the community, as possible.

Expanded Crisis Support (ECS)

Expanded Crisis Support or ECS is a program lasting up to 8 weeks for youth who have experienced a mental health crisis within the last 30 days and are not CAMHD involved. Youth can be referred to ECS by CMO, DOE, Emergency Shelters, social workers, medical providers. If a youth has not already been referred to ECS, KFGC staff can offer ECS when doing follow-up calls for non-CAMHD youth who have received a CMO outreach. There is an electronic referral form to complete. Youth must be between the ages 3-20yo and have experienced a mental health crisis in the last 30 days. Youth and family must agree to the service. Services include therapy (individual and family), care coordination, service plan development, and crisis prevention planning. The goal of ECS is to stabilize youth who have experienced a crisis and connect them with additional resources, such as CAMHD.

Child protection, prevention, and diversion services to keep children our of foster care. CWS services also include family support, foster care, adoption, legal guardianship, independent living, adoption assistance and guardianship payments, and licensing of resource caregivers, group homes, and child placing organizations.
